About This Disclosure

This page provides the telehealth disclosures required by Florida law for telehealth services provided by 5DGENX LLC and the licensed physicians in our network.

If you are a patient located in another state, additional state specific disclosures may apply. You will be provided with the relevant disclosures during your intake process.

What Is Telehealth

Telehealth is the delivery of healthcare services through electronic means, including:

• Online intake forms and questionnaires
• Review of laboratory and diagnostic results
• Video and phone consultations when applicable
• Secure messaging through the patient portal
• Electronic prescribing

You acknowledge that you are receiving healthcare services through telehealth and not through an in person visit.

Your Prescribing Physician

5DGENX maintains a network of licensed physicians. When you complete an intake, your case is assigned to a physician licensed in the state where you are located.

How to Verify Your Physician's License

The name and Florida license number (or applicable state license number) of your assigned prescribing physician will be provided to you in your patient portal and on prescription documentation.

You may verify any physician's Florida medical license through the Florida Department of Health at flhealthsource.gov.

For physicians licensed in other states, you may verify through the medical board of that state. Contact us if you need help locating the appropriate verification resource.

Scope of Telehealth Services

The 5DGENX telehealth practice provides:

• Initial intake review and clinical evaluation
• Laboratory result interpretation
• Protocol design and prescription
• Ongoing clinical supervision during programs
• Follow up evaluations

Services not provided through telehealth include:

• Emergency care (call 911 or visit the nearest emergency room)
• Procedures requiring in person presence (such as joint injections, IV infusions, and stem cell delivery)
• Diagnosis or treatment of acute conditions requiring in person examination

For services requiring in person delivery, you will be referred to a facility or provider where those services can be safely administered.

Your Right to In Person Consultation

You have the right at any time to:

• Request an in person consultation instead of or in addition to telehealth services
• Be referred to an in person provider in your area
• Decline telehealth services without affecting your future ability to receive care from us or another provider

If you prefer in person care, contact us at [email protected] and we will discuss your options. Note that some 5DGENX programs require telehealth based intake even if you ultimately receive in person procedural care.

Limitations of Telehealth

You acknowledge and understand that telehealth has limitations compared to in person care:

• The physician cannot perform a physical examination
• Some conditions may be difficult to diagnose without in person assessment
• Technical issues may interrupt or delay communication
• Not all medications or treatments are appropriate for telehealth prescribing

If the prescribing physician determines that your situation requires in person evaluation, they will recommend that you seek in person care before continuing with the program.

Privacy and Security in Telehealth

All telehealth communications between you and the 5DGENX team are conducted through HIPAA compliant platforms. Specifically:

• Patient portal communications are encrypted in transit and at rest
• Video consultations use HIPAA compliant video conferencing platforms
• Email communications are conducted through HIPAA compliant systems where PHI is involved
• All access to your records is logged and monitored

Emergency Situations

The following are considered medical emergencies:

• Chest pain or difficulty breathing
• Severe bleeding
• Sudden severe headache, vision changes, or weakness
• Severe allergic reaction
• Loss of consciousness
• Severe injury or trauma
• Thoughts of self harm or harm to others

If you experience any of these symptoms during or after a 5DGENX program, seek emergency care immediately and notify us as soon as you are able.

Consent to Telehealth

By using 5DGENX services, you consent to receiving healthcare services through telehealth as described in this disclosure. You acknowledge that you:

• Understand what telehealth is and how it will be delivered to you
• Understand your right to request in person care
• Understand the limitations and risks of telehealth
• Have had the opportunity to ask questions about telehealth and received satisfactory answers
