Telehealth is the delivery of healthcare services through electronic means, including:
You acknowledge that you are receiving healthcare services through telehealth and not through an in person visit.
5DGENX maintains a network of licensed physicians. When you complete an intake, your case is assigned to a physician licensed in the state where you are located.
The name and Florida license number (or applicable state license number) of your assigned prescribing physician will be provided to you in your patient portal and on prescription documentation.
You may verify any physician's Florida medical license through the Florida Department of Health at flhealthsource.gov.
For physicians licensed in other states, you may verify through the medical board of that state. Contact us if you need help locating the appropriate verification resource.
